Transition Planning

Transitions can be challenging for many young people, and for a child or young adult with a disability, it can often be complex and may require pre-planning and involvement from multiple sources. Whether your family is looking to create an Independent Education Plan (IEP), looking for information on scholarships for post-secondary education, securing a volunteer or paid position or applying for financial support, the articles and resources below will provide tips for their success and will help to identify ways to prepare them (and you) for life as a young adult.

Transition Planning from High School

TD Secure Futures and the RBC Future Launch Programs both aim to support youth with physical disabilities. With their generous financial support, Easter Seals Ontario was able to provide content, instruction and practical support to youth in an effort to prepare them as they transition from high school to post-secondary education.

Important Notice: Equipment Funding Program Availability (as of August 28, 2026)

The Equipment Funding Program (EFP) at Easter Seals Ontario is funded through the generosity of individual donors, fundraising events, corporate partners, and charitable foundations. As outlined in our program guidelines, funding assistance of up to $3,000 per eligible child is dependent on annual fundraising revenue and the availability of funds.

At this time, we have received a higher volume of funding requests than the funding currently available. While we will continue to accept requests for equipment funding, approvals will be granted in date received order and only as funding becomes available. As a result, applicants may experience longer-than-usual processing and decision timelines. While all eligible applications will be considered, funding approval cannot be guaranteed.

We understand that many families depend on this funding to obtain essential equipment and recognize the challenges that delays may create. Please be assured that our team continues to actively raise funds so that we can assist as many children and youth with physical disabilities as possible.

We appreciate your patience, understanding, and continued support of Easter Seals Ontario as we work to meet the growing demand for equipment funding assistance.
